Talasemia merupakan kelainan darah bawaan (keturunan) yang
diakibatkan adanya gangguan sintesis hemoglobin di dalam sel darah merah.
Untuk itu pada pasien talasemia diperlukan transfusi darah rutin. Transfusi
darah harus diberikan jika kadar Hb kurang dari 7 g/dL. Volume darah dan
frekuensi transfusi darah tergantung pada kadar Hb pasien. Setelah 2-4
minggu pasien diharapkan untuk kembali melakukan transfusi darah.
Transfusi darah rutin menyebabkan terjadinya penumpukkan zat besi di
berbagai organ, terutama hati dan jantung, sehingga menyebabkan kadar
Aspartate aminotranasferase (AST) dapat meningkat pada pasien talasemia.
Tujuan penelitian ini adalah untuk melihat perbandingan kadar AST pasien
talasemia berdasarkan frekuensi transfusi darah. Penelitian ini menggunakan
jenis penelitian survei analitik. Subjek penelitian berjumlah 30 orang. Teknik
pengambilan sampel menggunakan teknik accidental sampling. Hasil
penelitian memperlihatkan sebanyak 10 orang (33%) subjek penelitian
memiliki kadar AST di atas normal, 7 orang diantaranya melakukan transfusi
2 minggu sekali dan 3 orang lainnya melakukan transfusi setiap 3 minggu
sekali. Subjek penelitian yang melakukan transfusi setiap 4 minggu sekali
seluruhnya memiliki kadar AST normal. Uji One Way Anova menunjukkan
terdapat perbedaan bermakna paling tidak antar 2 kelompok (p = 0,043). Pada
uji post hoc LSD menunjukkan terdapat perbedaan bermakna pada kelompok
transfusi setiap 2 minggu sekali dan 4 minggu sekali (p = 0,013). Disarankan
bagi peneliti yang akan melanjutkan penelitian untuk melakukan penelitian
tentang pengaruh obat kelasi besi terhadap kadar AST pasien talasemia.

COMPARISON OF ASPARTATE AMINOTRANSPHERASE (AST)

BASED ON BLOOD TRANSFUSION FREQUENCY IN MAJOR β-
THALSEMIA PATIENTS IN IDAMAN REGIONAL HOSPITAL,

BANJARBARU

Thalassemia is a congenital blood disease caused by the inability of the
bone marrow to produce hemoglobin. Therefore, thalassemia patients need routine
blood transfusions. Blood transfusion should be given if the Hb level is less than 7
g/dL. Blood volume and frequency of blood transfusions depend on the Hb level
of thalassemia patients with the target Hb after transfusion being above 10 gr/dL
and not more than 14-15 gr/dL. The patient must return after 2-4 weeks for
another blood transfusion. On the other hand, routine blood transfusions cause
iron accumulation in various organs, especially the heart and heart, causing
Aspartate Aminotransferase (AST) levels to increase in people with thalassemia.
The purpose of this study was to compare AST levels in thalassemia patients
based on the frequency of blood transfusions. This study uses a type of analytic
survey research. The research subjects were 30 people. The sampling technique
used accidental sampling technique. The results of the study revealed that as many
as 10 people (33%) of the study subjects had AST levels above normal, 7 of them
transfused every 2 weeks and 3 other people had transfusions every 3 weeks. The
study subjects who were transfused once every 4 weeks all had normal AST
levels. The One Way Anova test showed that there was a significant difference
between at least the 2 groups (p = 0.043). Then the LSD post hoc test showed that
there was a significant difference in the transfusion group every 2 weeks and once
every 4 weeks (p = 0.013). It is recommended for researchers who will continue
their research to conduct research on the effect of chelation drugs on AST levels
in thalassemia patients.
