Tuberkulosis (TB) adalah salah satu penyakit infeksi tertua yang melekat
sepanjang sejarah peradaban manusia dan masih menjadi masalah kesehatan
masyarakat.Tes Cepat Molekuler (TCM) merupakan alat diagnosis utama yang
mempunyai sensitivitas dan spesifisitas yang jauh lebih baik untuk diagnosis TB
dibandingkan pemeriksaan mikroskopis serta mendekati kualitas diagnosis
dengan pemeriksaan biakan. Merokok tembakau dapat mempengaruhi sistem
kekebalan tubuh dengan menyebabkan defek pada sel imun seperti makrofag,
monosit dan limfosit CD4 (kluster diferensiasi 4). Tujuan penelitian untuk
mengetahui pengaruh frekuensi merokok dengan kejadian TB pada pasien rawat
inap di ruang isolasi RSUD Datu Sanggul. Jenis penelitian Survey Analitik dengan
rancangan penelitian Cross Sectional. Populasi penelitian ini adalah pasien TB
yang melakukan pemeriksaan TCM di ruang rawat inap isolasi RSUD Datu
Sanggul periode 15 Juni – 15 September 2023 sebanyak 45 sampel. Pengambilan
sampling dengan teknik total sampling. Hasil penelitian didapatkan hasil tidak
terdeteksi MTB sebanyak 15 sampel, dan terdeteksi MTB sebanyak 30 sampel (5
hasil MTB deteceted very low, 11 hasil MTB detected low, 6 hasil MTB detected
medium, 8 hasil MTB detected high). Lembar kuesioner menunjukkan >50%
responden belum mengetahui tentang pengetahuan dan faktor resiko, dan
hubungan frekuensi lama merokok dari hasil lembar kuesioner tentang lama
merokok di dapatkan <10 tahun 2 orang (8%) dam >10 tahun di dapatkan 24
orang (92%) dengan klasifikasi merokok ringan 2 orang (8%), klasifikasi sedang
16 orang (62%), klasifikasi berat 5 orang (19%), dan klasifikasi sangat berat 3
orang (11%). Saran untuk peneliti selanjutnya dapat meneliti hubungan pada
kategori pasien berbeda seperti terduga TB dengan riwayat penyakit bawaan
seperti HIV/AIDS, DM, ginjal kronis.

ABSTRACT

THE RELATIONSHIP BETWEEN THE FREQUENCY OF SMOKING
AND THE OCCURRENCE OF TUBERKULOSIS (TB) IN INPATIENTS IN
THE ISOLATION WARD OF DATU SANGGUL REGIONAL HOSPITAL

Tuberculosis (TB) is one of the oldest infectious diseases in human history and
remains a public health problem. The Molecular Rapid Test (MRT) is the main
diagnostic tool with better sensitivity and specificity for diagnosing TB compared
to microscopic examination and approaches the quality of diagnosis with culture
examination. Tobacco smoking can affect the immune system by causing defects in
immune cells such as macrophages, monocytes, and CD4 lymphocytes (cluster of
differentiation 4). The purpose of this study was to determine the effect of smoking
frequency on the occurrence of TB in inpatients in the isolation ward of Datu
Sanggul Regional Hospital. This was an analytic survey with a cross-sectional
research design. The study population consisted of TB patients who underwent
MRT examination in the isolation ward of Datu Sanggul Regional Hospital from
June 15th to September 15th, 2023, with a total of 45 samples. Sampling was done
using total sampling technique. The results showed that TB was not detected in 15
samples, while it was detected in 30 samples (5 had very low TB detection, 11 had
low TB detection, 6 had medium TB detection, and 8 had high TB detection). The
questionnaire sheets indicated that >50% of respondents did not have knowledge
about risk factors and the relationship between smoking frequency and TB
occurrence. From the questionnaire, it was found that 2 individuals (8%) had
smoked for less than 10 years, while 24 individuals (92%) had smoked for more
than 10 years, with 2 individuals classified as light smokers (8%), 16 as moderate
smokers (62%), 5 as heavy smokers (19%), and 3 as very heavy smokers (11%). It
is suggested that future research should investigate the relationship in different
patient categories, such as suspected TB patients with underlying conditions like
HIV/AIDS, diabetes mellitus, or chronic kidney disease.
