Penderita Diabetes Melitus (DM) tipe II berisiko mengalami komplikasi 
Penyakit Arteri Perifer (PAP) yang dapat menyebabkan gangguan perfusi perifer 
hingga amputasi. Salah satu upaya non-farmakologis untuk meningkatkan sirkulasi 
darah perifer adalah dengan melakukan Buerger Allen Exercise (BAE). Latihan ini 
melibatkan perubahan postur, penyesuaian gravitasi untuk merangsang sirkulasi 
perifer, dan peningkatan aliran darah ke ekstremitas bawah melalui kontraksi otot.
Tujuan karya tulis ilmiah ini adalah untuk melaksanakan asuhan keperawatan pada 
pasien diabetes melitus tipe II dengan masalah keperawatan perfusi perifer tidak 
efektif melalui implementasi BAE.
Metode yang digunakan adalah studi kasus deskriptif dengan pendekatan 
proses keperawatan pada dua responden di wilayah kerja Puskesmas Landasan Ulin 
Timur. Fokus utama asuhan keperawatan yaitu masalah perfusi perifer tidak efektif 
pada penderita DM tipe II. Intervensi yang diberikan berupa BAE selama 15 menit, 
dua kali sehari, selama lima hari berturut-turut. Pengumpulan data dilakukan 
melalui wawancara, observasi, pemeriksaan fisik, studi literatur dan studi 
dokumentasi.
Hasil menunjukkan adanya peningkatan status perfusi perifer, ditandai 
dengan meningkatnya denyut nadi perifer dan sensasi, menurunnya warna kulit 
pucat, nyeri ekstremitas, parestesia, dan membaiknya pengisian kapiler, turgor 
kulit, serta kondisi akral. Pasien merespons baik terhadap latihan yang diberikan 
dan tidak ditemukan komplikasi selama pelaksanaan.
Kesimpulan dari penelitian ini adalah implementasi BAE efektif dalam 
mengatasi masalah perfusi perifer tidak efektif pada penderita DM tipe II. Saran 
bagi tenaga keperawatan agar menjadikan BAE sebagai intervensi mandiri untuk 
meningkatkan vaskularisasi perifer penderita diabetes.

Patients with Type II Diabetes Mellitus (DM) are at risk of developing 
Peripheral Arterial Disease (PAD) complications that may lead to impaired 
peripheral perfusion and even amputation. Buerger Allen Exercise (BAE) serves as 
a non-pharmacological intervention to improve peripheral blood circulation. This 
exercise involves postural changes and gravitational positioning to stimulate 
peripheral circulation while enhancing blood flow to lower extremities through 
muscle contraction. This scientific paper aims to implement nursing care for type 
II DM patients with ineffective peripheral perfusion through BAE implementation.
The study employed a descriptive case study method using a nursing 
process approach on two respondents in the working area of Landasan Ulin Timur 
Community Health Center. The primary focus was addressing ineffective 
peripheral perfusion in type II DM patients. The intervention consisted of BAE 
conducted for 15 minutes, twice daily, for five consecutive days. Data were 
collected through interviews, observation, physical examination, literature review, 
and documentation study.
Results demonstrated improved peripheral perfusion status, evidenced by 
enhanced peripheral pulses and sensation, reduced skin pallor, extremity pain, 
paresthesia, as well as improved capillary refill, skin turgor, and acral condition. 
Respondents responded well to the exercise with no complications observed during 
implementation.
The study concludes that BAE implementation is effective in managing 
ineffective peripheral perfusion among type II DM patients. Nursing practitioners 
are advised to adopt BAE as an independent intervention to enhance peripheral 
vascularization in diabetic patients.
