Demam merupakan kondisi umum pada anak yang memerlukan 
penanganan segera untuk mencegah komplikasi, seperti kejang. Salah satu metode 
penanganan nonfarmakologis tradisional yang digunakan masyarakat adalah 
kompres bawang merah. Bawang merah mengandung senyawa aktif seperti sulfur 
dan minyak atsiri yang dipercaya menurunkan suhu tubuh melalui vasodilatasi dan 
evaporasi. Penelitian ini bertujuan mengeksplorasi penerapan kompres bawang 
merah dalam asuhan keperawatan anak demam di UPTD Puskesmas Martapura 2.
Penelitian ini merupakan studi kasus dengan pendekatan proses 
keperawatan pada dua anak usia 1–5 tahun yang mengalami demam >37,5°C tanpa 
komplikasi dan alergi bawang merah. Data diperoleh melalui wawancara, 
observasi, pemeriksaan fisik, dan dokumentasi, dilaksanakan di UPTD Puskesmas 
Martapura 2 pada Maret–April 2025. Analisis dilakukan secara deskriptif melalui 
lima tahap proses keperawatan.
Hasil menunjukkan penurunan suhu tubuh yang signifikan setelah 
implementasi kompres bawang merah selama tiga hari. Pada An. U dengan suhu 
tubuh 38,6°C, terjadi penurunan suhu menjadi 36,6°C. Sementara itu, pada An. K 
dengan suhu awal 38,9°C, suhu juga mengalami penurunan menjadi 36,8°C. 
Kondisi umum kedua anak membaik, dan tidak ditemukan efek samping. Orang tua 
juga mampu melanjutkan tindakan secara mandiri.
Kompres bawang merah efektif sebagai intervensi nonfarmakologis untuk 
menurunkan demam anak. Metode ini aman, mudah diterima masyarakat, dan dapat 
dilakukan di rumah dengan bimbingan perawat. Perawat diharapkan 
mengintegrasikan terapi ini dalam praktik keperawatan komunitas dan 
meningkatkan edukasi kepada keluarga.

Fever is a common condition in children that requires prompt treatment to 
prevent complications such as seizures. One traditional non-pharmacological 
method commonly used in the community is the red onion compress. Red onions 
contain active compounds such as sulfur and essential oils that are believed to 
reduce body temperature through vasodilation and evaporation. This study aims to 
explore the implementation of red onion compress in nursing care for children with 
fever at UPTD Puskesmas Martapura 2.
This research is a case study using a nursing process approach involving 
two children aged 1–5 years who experienced fever (>37.5°C) without severe 
complications or red onion allergies. Data were collected through interviews, 
observation, physical examination, and documentation. The study was conducted 
at UPTD Puskesmas Martapura 2 from March to April 2025 and analyzed 
descriptively through the five stages of the nursing process.
The results showed a significant decrease in body temperature following 
the implementation of red onion compresses over a three-day period. In the case of 
patient U, the body temperature decreased from 38.6°C to 36.6°C. Similarly, 
patient K’s temperature decreased from 38.9°C to 36.8°C. Both children’s general 
condition improved, and no side effects were observed. Parents were also able to 
continue the intervention independently.
Red onion compresses proved effective as a non-pharmacological 
intervention to reduce fever in children. This method is safe, well-accepted in the 
community, and feasible for home use with appropriate guidance from nurses. It is 
recommended that nurses integrate this therapy into community nursing practice 
and enhance family education regarding its techniques and indications.
